Call 1-800-456-3660. Open accounts are available (subject to approval) to all licensed practitioners in the United States. Please provide doctor's name, address, phone number and social security number.

Pricing
At Star Dental Supply you will always receive our lowest possible price at time of purchase. Star Dental strives to maintain our pricing for the duration of this catalog, increasing prices only in response to manufacturer increases. While every effort has been made to insure accuracy, Star Dental can not be held responsible for typographical errors.

Payment Terms
Open account - pay from statement. Invoices 30 days past due will be charged a 1.5% service charge per month. All major credit cards accepted.

Credit and Return Policy
Items purchased from Star Dental Supply may be returned forcredit within 30 days of the invoice date. Merchandise returned after 30 days will incur a 10% restocking fee. A copy of the invoice must accompany all retuned goods. Products returned must be in their original, sealed package. If a product is defective return the unused portion along with an explanation for the manufacturer. Any equipment, including handpieces, if returned opened or used, will be repaired or replaced according to the manufacturer's warranty and are NOT returnable for credit or exchange.

Delivery information
Local (50 mile radius):
Same day delivery available for all local orders (subject to a $25 fee). Next day delivery for all local orders are subject to $4.95 shipping and handling fee ($10 fee for all orders under $100). Orders must be received by 5pm PST.

Outside Local Area:
Minimum of $9.95 shipping and handling for all orders within the continental US.

Special Orders
While Star Dental maintains a wide array of merchandise, occasionally we may not have a specific item in stock. All efforts will be made to provide that item in the fastest way possible. Some restrictions or special handling fees may apply.
